Please note below that the Community Mental Health Assessment application will remain offline for security enhancements and is expected to be restored on 4/26/16. The state is encouraging Assessors to complete paper versions of these assessments in order to support individuals access to HCBS services until the system is totally up and running again. See below for details.

We appreciate your continued patience while the Community Mental Health Assessment application remains offline for security enhancements. The system in expected to be restored on 4/26/16. When the system is restored, the security enhancements will require all users to complete a multifactor authentication process.Please see the attached documents for instructions and more information. Please note, users will not be able to access the system until this process is completed.
Until the application is back online, Assessors are encouraged to use the paper versions of the HARP/HCBS Eligibility Assessment and Community Mental Health Assessment. PDF versions of these Assessments can be accessed in the “Assessment Instrument” section of 8100 Community Mental Health Assessment. This section also includes instructions on how to manually score the HARP/HCBS Eligibility Assessment. Once the system is restored, all completed paper assessments need to be entered into the system and signed as per existing procedures.
