NYAPRS Note: Please see below for an opportunity to join the National Council of Behavioral Health and their partners in the Care Transitions Network, a four-year project funded by CMS to reduce all-cause re-hos­pitalization rates for people with serious mental health issues. The Care Transitions Network offers targeted coaching, clinical consultation, and organizational trainings and resources to aid in implementing evidence-based practices and transition to value-based payments, short-term care transitions support to facilitate and ensure engagement with outpatient practices for people post discharge from psychiatric hospitalizations and a web-based platform for Quality Improvement to enable providers to track progress on key quality indicators.
