OMH Survey Close Date Extended: Survey on Understanding Attitudes, Beliefs, and Behaviors – for Mental Health Service Recipients

Thank you to all of you that already submitted responses. This is a reminder that the OMH is still collecting responses for this survey from individuals who receive services from any OMH licensed, funded, or designated program. Please disseminate as appropriate.

If responding to the survey online, please click the survey link or scan the QR code only when you are ready to take the survey as duplicate responses will be blocked.

If you are looking to submit responses from different service recipients using the same device, please connect with us at omhippi@omh.ny.gov. Separate survey links can be provided for this purpose on an as needed basis. If you like to look back at your previous responses, please click ‘Back’ found at the bottom of each page of the survey interface. Kindly do not use the ‘Back’ option of the browser for this function. Please click ‘Submit’ to submit your responses after completing the online survey. 

QR CODE FOR THE SERVICE RECIPIENT SURVEY:

We encourage providers to offer the survey on paper if a recipient cannot access the online survey versions (see attached).  Following completion of a paper survey, we ask that programs or individuals enter the recipient’s responses to the online platform. To be able to enter data in bulk using the same device, please connect with us at omhippi@omh.ny.gov. Separate survey links will be provided for this purpose on an as needed basis. This allows OMH to receive the responses most efficiently. 

If that’s not possible, you can scan the paper surveys and email to omhippi@omh.ny.gov or fax to 518-408-2210.  Please ensure that no personal health information is on the form. 

The survey is expected to close on July 5, 2022.

The Office of Mental Health is seeking broad input about attitudes, beliefs, and behaviors related to mental health.  More specifically, the following survey seeks to obtain this input from people who receive services.

Survey responses are anonymous and only aggregate data will be shared. Findings will help depict the current state, of attitudes, beliefs, and behaviors here in New York.  Findings will also guide the creation of interventions to help promote affirming attitudes. 

The Office of Mental Health recognizes that some individuals receiving services may have trouble accessing the survey, so we’re asking service providers, advocates, and other supporters to reach out to individuals and complete the survey with them.  In doing so, please make sure that ONLY the information of the individual you are assisting is included in the responses.
