The CORE Peer Navigator Project is a statewide initiative that provides peer support while connecting individuals to mental health resources and community services of their choice. A simple call to our toll-free Navigator Line at 855-PEERNAV (855-733-7628), connects an individual to a Peer Navigator in their community.
What We Offer:
- Personalized Support: Tailored guidance based on the person’s unique needs and circumstances.
- Connection to Services: Assistance in navigating healthcare, housing, employment, and more.
- Empathy and Understanding: Peer Navigators provide support from a place of shared experience.
How It Works:
- An individual can call the Navigator Line (855-PEERNAV) on their own or with a provider.
- A short phone call will provide additional project information and a connection to a local Peer Navigator who can meet via text, phone call, Zoom, or in-person.
- The Peer Navigator will work with the participant to discuss their needs and goals.
- The Peer Navigator will assist the participant in verifying their Health and Recovery Plan (HARP) eligibility through Managed Medicaid and share what OMH Community Oriented Recovery and Empowerment (CORE) Services are available to them.
- The participant will get connected to the services and supports that they choose and receive regular check-ins and continuous guidance to ensure their needs are being met.
Did You Know?
The Alliance’s CORE Peer Navigator Project has materials and supports available for Spanish-speaking participants. The same simple phone call to our toll-free 1-855-PEERNAV phone line gets an individual connected to a Spanish-speaking CORE Peer Navigator. Their CORE Peer Navigator can assist them in finding an OMH CORE service provider in their community that offers
services in Spanish.
