NYAPRS Note: The update below from DOH regarding a new Health Information Technology website is particularly important as Health Homes begin to position themselves for care management responsibilities in a managed care environment. HIT transparency is valuable for consumers, providers, and MCOs to determine readiness and effectiveness, as well as accountability to new standards and outcomes. There is also a new Personal Health Information (PHI) sharing policy; advocates must be vigilant about reviewing these policies and the implementation thereof in order to continually protect the rights of consumers.
DOH: The Health Home Program has created a new web page covering Health Information Technology (HIT). This web page currently details HIT Standards and progress of all Health Homes as of December 2013. This new section be updated periodically.
To view the new HIT section visit this web page:http://www.health.ny.gov/health_care/medicaid/program/medicaid_health_homes/health_info_technology.htm
Also, be advised that the Health Home Program has formulated a revised policy for sharing protected health information of Health Home enrollees.
The updated policy can be accessed by visiting this web page: http://www.health.ny.gov/health_care/medicaid/program/medicaid_health_homes/forms/docs/policy_for_sharing_protect_hlth_info.pdf
