The Peer Support Mentor position is:
Based at the Empowerment Exchange in Troy, New York. The Empowerment Exchange is a peer support program dedicated to offering strategies that encourage people throughout Rensselaer County to use their personal power and the power of peer support to move forward in their lives.
Responsibilities of this position include:
- Working with people in both one-to-one and group settings, facilitating classes, events and activities.
- Helping people establish creative community connections and assisting them to develop natural supports;
- Establishing connections to community based activities that enhance wellness, recovery and social engagement.
- Providing individualized peer-to-peer outreach and support in their homes and other settings to people who may find it difficult to reach out and connect to their communities
- Engaging people in one on one recovery planning, using tools such as Wellness Recovery Action Planning and Intentional Peer Support; Assisting people to choose, record and achieve wellness goals;
- Offering individualized advocacy and support services to help a person learn about and protect their rights and interests. This may include assisting people to work through system processes and procedures; researching information, preparing documents, role-playing situations and attending meetings as needed;
- Organizing and coordinating wellness/recovery events and activities.
- Mentoring volunteers
- Any duties deemed appropriate to achieve the goals of the project.
Qualifications The ideal candidate:
- Will be an active team member, offering support and assistance to ongoing projects.
- Will be able to work independently on projects, meeting deadlines and taking initiative.
- Will have basic computer skills; competence in Microsoft Office required.
- Will demonstrate knowledge of job duties through follow-up and accurate record keeping is also important.
A clean, NYS Driver’s License and reliable transportation is also required.

Part-Time Peer Advocate in Baltic St. Bronx Peer Advocacy Program
Baltic Street AEH, Inc., seeks applicants for the position of Peer Advocate. The person will perform advocacy services for and with consumers of mental health services in a program that provides a wide network of Peer Advocacy services
One Part-time – 20 hrs
Essential Functions:
- Provide linkages to services including Mental Health, Legal, Housing, Education, Medical, Social, and Financial Services, etc.
- Aid recipients to obtain services for themselves.
- Facilitate and co-facilitate recovery and self-help groups, which includes sharing one’s own recovery story/experience with consumers, and functioning as a role model exhibiting competency in personal recovery.
- Work with clients via phone and walk-in.
- Persist and follow through until cases are complete.
- Outreach to communities.
Qualifications:
- Experience as a Peer Advocate, or other related experience.
- Knowledge of self-help techniques.
- Basic working knowledge of mental health services.
- Good reading and writing Skills.
- Clean drivers license a plus.
- Office and computer skills are required.
- Bilingual a plus

Venture House, an innovative psychiatric rehabilitation program seeks a highly motivated, energetic, creative individual to become part of our team.
Venture House is a Clubhouse International Accredited Program (www.iccd.org). Staff Generalists are responsible for organizing tasks and projects (i.e. Clubhouse newsletter, preparing meals, data entry, maintenance, gardening), while incorporating members (our clientele) into the work of the Clubhouse. In addition Staff Generalists are also responsible for managing a member list (caseload) of up to 25 people along with providing other services as needed including; housing, employment and the social/recreation program. Staff is responsible for writing service plans and progress notes.
Staff Generalists also provide assistance to people with mental illness with regard to entitlements, housing, advocacy, employment & education and anything else needed to assist the member in meeting their goals. All work is carried out in a manner that is person centered with a focus on recovery from psychiatric illness/experience and/or co-occurring disorders (CoD).
Flexibility and a belief in rehabilitation and recovery are essential. A BA/BS is required. Experience in an accredited Clubhouse is a plus as are bilingual skills. People with a lived experience with mental illness are encouraged to apply. Drivers’ license required.

Disparities Project Coordinator for THINC Population Health Improvement Program

The Institute operates 27 full and part time health centers throughout the Bronx, Manhattan and the mid-Hudson Valley region of New York State. Summary of Job Posting:
THINC has subcontracted with the Institute for Family Health to lead the health disparities improvement activities that are part of New York State Department of Health’s Population Health Improvement Program (PHIP). The project will cover a seven county region of the Hudson Valley to provide regional convening, capacity building and data services.
The goal of the project is to promote the Triple Aim of better care, better population health and lower health care costs. THINC will provide coordination and oversight of the implementation of PHIP in the Hudson Valley and coordinate with other staff on program execution. The Institute’s Disparities Project Coordinator will work closely with the THINC Program Manager and Prevention Agenda Coordinator and the project steering committee.
The Coordinator will focus on the assessment of health disparities issues as they relate to population health improvement activities of local health departments, hospitals and health care consumers; coordination of organization-wide consumer engagement strategies; and support of organization-wide program evaluation efforts. S/he will work with other PHIP participant to:
- Convene a Prevention Agenda/health disparities workgroup representing target communities
- Assess needs and priorities for target communities
- Determine strategies to address priorities
- Provide or arrange technical assistance and/or training to as identified in strategic priorities
- Evaluate impact of technical assistance for health disparities
- Disseminate information about outcomes of technical assistance
Specifically, this will include:
- Reviewing needs assessments and other local and countywide data to identify areas of disparities, and identifying overlap with other prevention agenda issues,
- Coordinating steering committee and stakeholder meetings,
- Coordinating technical assistance and training
- Designing and coordinating consumer engagement activities as appropriate
- Supporting PHIP workgroups as needed, including: health disparities, prevention agenda, and ad hoc groups as formed;
- Providing support for evaluation needs, including evaluation design, data mining and reporting;
- Demonstrating flexibility and adjust to shifting priorities, demands and timelines through problem solving capabilities;
- Collaborating with PHIP staff at Southern Tier Health Link as well as PHIP contractors across New
York State;
- Other duties as assigned.
