The Health Home Program has organized two Town Hall Meetings for Designated Health Homes (meaning the lead entity and a representation of  their partners).  Each Lead Health Home has been asked to develop a list of representatives from their Health Home partnerships, including transitioning care management programs (TCM,MATS,CIDP) and other providers delivering Health Home services directly. The number of attendees that can be present is limited, but  DOH will webcast both meetings.  

Health Home partners should inform their lead Designated Health Home provider of their interest to attend and participate in these meetings.

DOH recommends that those not able to attend in person, should submit questions to the lead Designated Health Home provider so they can ask them at the Town Hall meeting.

 The information that was presented at the June 6th, 2012 statewide webinar will be presented again leaving the majority of time for questions and discussion.

Information on how to attend a Town Hall meetings via webcast will be forth coming in a listserv email and will be posted on the Health Home website once finalized under the What’s New section of the Health Home website at this URL:
