Statewide Multicultural Advisory Committee
Membership Application
(Applications Due – July 1, 2021)
Since 1989 OMH has made use of the valuable input of the SMAC, a 25-member advisory committee whose membership includes consumers of mental health services, experts in the field, policy makers, and researchers. Members advise the Commissioner and OMH on policy, programs, procedures, and activities addressing disparities in access, quality, and outcomes for members of historically underserved, disadvantaged, and marginalized populations.
To apply for membership, please complete the following three steps:
1. Provide a signed and dated cover letter that includes the following information:

Race
Ethnicity
Language(s) Spoken
Profession/Employment
Awards/License/Certifications
2. Include a 1-page response answering the following question: “Why do you want to be a member of the NYS OMH Statewide Multicultural Advisory Committee?”
3. Include a current resume containing information relevant to the selection process.
The completion of these steps will constitute your application for membership to the Statewide Multicultural Advisory Committee. Once all applications are reviewed, recommendations will be made to the New York State Commissioner of Mental Health. You will be contacted regarding your application status once the membership selection has been completed.
