UPDATED Phase-in plan for Health Home Applications
Phasing in Health Home Implementation
NYS Department of Health January 10, 2012
- Phase I – 10 counties:
- Bronx, Clinton, Kings (Brooklyn), Essex, Franklin, Hamilton, Nassau, Schenectady, Warren, Washington
- HH application due date for Phase I counties only is November 1, 2011.
- Implementation is scheduled for February 1, 2012
- Phase II** – 16 Counties:
- Albany, Dutchess, Erie, Manhattan, Monroe, Orange, Putnam, Queens, Rensselaer, Richmond (Staten Island), Rockland, Saratoga, Suffolk, Sullivan, Ulster, Westchester,
- HH application due date for Phase II counties only is February 1, 2012.
- UPDATED Implementation is tentatively scheduled for April 1, 2012.
- Phase III** – 36 Counties:
- Alleghany, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Columbia, Cortland, Delaware, Fulton, Genesee, Greene, Herkimer, Jefferson, Lewis, Livingston, Madison, Montgomery, Niagara, Ontario, Oneida, Onondaga, Orleans, Oswego, Otsego, Schoharie, Schuyler, Seneca, St. Lawrence, Steuben, Tioga, Tompkins, Wayne, Wyoming, Yates
- HH application due date for Phase III counties only is April 21, 2012.
- UPDATED Implementation is tentatively scheduled for July 1, 2012.
- **Please note that Health Homes approved to provide Health Home services under Phase I will need to resubmit their application with updated information for Phase II and III if they plan to provide Health Home services to Medicaid enrollees in these counties. Phase 1 applicants that desire to import a copy of the information in their Phase I application to a new Phase II application, should send a note requesting assistance to the Health Home Mailbox hh2011@health.state.ny.us. with the subject: Import Phase 1 application.
