The New York State Department of Health will be presenting a webinar On February 28, 2012 entitled “Tools for Implementation of Health Homes.”
The discussion will include:
- an explanation of the roles of designated Health Homes and Managed Care Plans,
- the steps that need to be taken to implement the new program,
- an overview of model contract language,
- how to bill and receive payment for services,
- the care management and quality metrics used for reporting; and
- a self assessment tool that will be used for evaluation.
