Dear Agency Partner,
I am excited to announce that the JFK, Jr. Institute for Worker Education at CUNY, in association with Queensborough Community College, will offer an undergraduate certificate in Health Coaching and Care Coordination beginning this summer 2012. This 10-credit, 5 course certificate is offered to frontline workers in the allied health fields who have a GED or High School Diploma, are residents of NYS, and are available to commit to completing a sequence of 5 courses (a college bridge course and 4 credited courses) through summer 2013.
The attached application packet includes detailed program information, eligibility criteria, and required forms. The application can also be accessed on our website: http://cuny.edu/about/administration/offices/hhs/hps-program/Summer2012.CCHCApplication.pdf. CUNY will conduct rolling admissions and applications will be considered on a priority basis. All applications must be submitted no later than Friday, May 4th.
Please share this exciting opportunity with staff members at your agency who may be interested and please don’t hesitate to contact me with any questions. As with our past programs, this will be a similarly competitive admissions process. I am happy to speak directly with potential applicants who have questions regarding the program or eligibility. Institute representatives are also available to come to your agency and present the Certificate to staff- please contact me if you would like to schedule a visit.
Regards,
Abigail
