DOH Announces Phase II Health Home Designees

NYAPRS Note: The NYS Department of Health announced the following health home designees in Phase II counties: Dutchess, Erie, Manhattan, Monroe, Orange, Putnam, Queens, Richmond (Staten Island), Rockland, Suffolk, Sullivan, Ulster, Westchester.

In July, DOH is expected to announce designees for Phase III counties including Albany, Alleghany, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Columbia, Cortland, Delaware, Fulton, Genesee, Greene, Herkimer, Jefferson, Lewis, Livingston, Madison, Montgomery, Niagara, Ontario, Oneida, Onondaga, Orleans, Oswego, Otsego, Rensselaer, Saratoga, Schoharie, Schuyler, Seneca, St. Lawrence, Steuben, Tioga, Tompkins, Wayne, Wyoming, Yates.

Health homes are already up and running in Phase I counties including Bronx, Brooklyn, Nassau, Schenectady and Northern Region counties Clinton, Essex, Franklin, Hamilton, Warren and Washington.

 

Phase II Designated Health Homes

The NYSDOH received 42 Health Home applications to serve the thirteen counties in Phase II. The applicants listed below are being contingently designated as Provider-led Medicaid Health Homes and must meet specified contingencies before receiving final approval. The contingently designated Health Homes by region are:

 

Central- Monroe

  • Anthony L. Jordan Health Center- on behalf of Greater Rochester Health Home Network, LLC
  • Huther Doyle Memorial Institute, Inc.

 

Western – Erie

  • Alcohol and Drug Dependency Services, Inc.
  • Mental Health Services Erie County, SE Corp V
  • Urban Family Practice, P.C.

 

Hudson Valley – Dutchess, Orange, Putnam, Rockland, Sullivan, Westchester, Ulster

  • Hudson River HealthCare, Inc.
  • Open Door Family Medical Center, Inc- on behalf of a new joint venture known as The Hudson Valley Care Coalition (HVCC) with Hudson Valley Community Services, Inc., The Greater Hudson Valley Family Health Center, Inc., Mental Health Association of Rockland County, Inc., Rehabilitation Support Services, Inc.
  • Institute for Family Health (Ulster County)

 

Long Island – Suffolk

  • FEGS Health & Human Services System
  • Hudson River HealthCare, Inc.
  • North Shore – LIJ Health System

 

New York City- Queens

  • Community Healthcare Network
  • New York City Health and Hospitals Corporation
  • North Shore – LIJ Health System

 

New York City- Richmond (Staten Island)

  • Jewish Board of Family and Children’s Services

 

New York City- Manhattan
