Integrated Care Resource Center Offers New Ways to Search for Online Resources

The Integrated Care Resource Center – a Centers for Medicare & Medicaid Services initiative to help states establish integrated care programs for Medicare-Medicaid enrollees and for high-need, high-cost individuals via health homes – is pleased to introduce its redesigned website: IntegratedCareResourceCenter.com.

The website offers new features to help users more quickly find the information they need:

• An interactive map lets users search for information on specific states;
• New pages list states pursuing Medicaid health homes and integrated programs for Medicare and Medicaid beneficiaries; and
• Quick links on the home page connect to new resources, popular downloads, and more.

The compendium of resources on the Integrated Care Resource Center website, including issue briefs, practical tools, and templates, can help states design and implement new ways to improve care for Medicaid’s high-need, high-cost beneficiaries. Resources, including state best practice documents, are updated regularly to reflect the lessons gleaned from on-the-ground experiences of states pursuing integrated care models.
