The Collaborative Care Model: An Approach for Integrating Physical and Mental Health in Medicaid Health Homes

This Exploring Medicaid Health Homes webinar will highlight the Collaborative Care Model, one evidence-based approach to physical-mental health integration, for states to consider in developing Medicaid health homes. Participants will hear from Dr. Jürgen Unützer, the developer and lead researcher behind the Collaborative Care Model, as well as from a panel of stakeholders with experience implementing this model. The webinar will highlight details of the model, describe the evidence documenting its effectiveness, and describe how it operates from the perspective of primary care providers, specialty mental health providers, and payers.
I. Update on Health Home Activity Across the States
Speaker: Allison Hamblin, Vice President, Center for Health Care Strategies (CHCS)
Allison Hamblin will welcome participants and provide an update on health homes activities at a national level, with a focus on efforts to integrate physical and behavioral health services.
II. Overview of the Collaborative Care Model
Speaker: Jürgen Unützer, MD, MPH, Professor and Vice Chair, Psychiatry and Behavioral Sciences; Director, AIMS Center (Advancing Integrated Mental Health Solutions), University of Washington
Dr. Jürgen Unützer will describe the core elements of the Collaborative Care Model, an approach to improving integration of mental health in primary care settings. He will detail the evidence supporting the model’s effectiveness in improving cost and quality outcomes, and explain how the model has been successfully implemented at scale with safety-net providers.
III. Frontline Perspectives on Collaborative Care
