Collaborative Documentation, also known as Concurrent Documentation, is a process in which clinicians and recipients work together to build trust, and create a therapeutic alliance. By sharing the responsibility of documenting services together, the therapeutic sessions become much more meaningful for both parties. Collaborative Documentation is also a key component and catalyst towards Shared Decision Making.
Shared decision-making (SDM) is a practice and concept with the potential to advance wellness and recovery in mental health care. By making the consumer an indispensable partner in the process of recovery, SDM advances many of the goals of mental health care transformation, previously identified by the President’s New Freedom Commission on Mental Health, the Institute of Medicine, National Association of Mental Health Program Directors and the NYS Office of Mental Health.
The presenter will be Tony Trahan, Advocacy Specialist II at the NYS Office of Mental Health.
