NYAPRS Note: How do we break down the barriers to integration and start to truly work together in the delivery of new and innovative recovery oriented services? Join us as we hear from an exciting agency in Portland Oregon that began as a small agency responding to a gap in their local continuum of care and has now grown to become a successful model agency for true integration.
One of the factors in their success is their use of Team-based Care to drive planning and decision making and service delivery. Hear how this new and exciting method works for them.
OUR PRESENTERS:
Rachel Solotaroff, MD, MCR, is the Central City Concern (CCC) Medical Director and provides medical oversight to Central City Concern’s nine primary care and behavioral health clinics, as well as strategic leadership of the larger project of fully integrating the continuum of services provided by Central City Concern, including housing, behavioral health, and primary care, with the goal of creating seamless, comprehensive care to support clients in their journeys to self-sufficiency. She also is the co-director of the Oregon Health & Science University’s Social Medicine Curriculum for OHSU Internal Medicine residents, providing an opportunity for physicians-in-training to learn from the unique Central City Concern model of health care.
Leslie Tallyn, MA, is the CCC Director of Quality and Compliance and is responsible for overseeing process improvement, outcomes management, and internal evaluations, as well as strategic initiatives related to just culture and continuous quality improvement.
