Care Coordination in Action: Sharing Behavioral Health Patient Information
Date: Wednesday, November 20, 2 – 3:30 pm EST Speakers:Allison Hamblin, Vice President for Strategic Planning, Center for Health Care Strategies; David Mancuso, Director of Research and Data Analytic Division Services & Enterprise Support Administration, Washington State Department of Social and Health Services; Mike Lardieri, VP, Health Information Technology, National Council for Behavioral Health
Information sharing can lead to increased coordination of services, lowered costs, and improved quality of care for clients. In an integrated care setting, information sharing is critical. Current integrated care models include Accountable Care Organizations, Care Coordination Organizations, Medical and Health Homes. In the future, providers, hospitals and health plans will be pushed to participate and share data within integrated health systems and through some form of health information exchange (HIE).
Join the National Council and the Center for Health Care Strategies to learn real world applications on how to share information and implement HIEs for care coordination. Explore the current state of information sharing, review federal regulations that direct the exchange of behavioral health information, and discover local innovations among mental health and substance abuse providers.
