NYAPRS Note: Care Management is an emerging concept that refers to integrated clinical care activities tailored to each individual. Case managers have transitioned to Health Home Care Managers and are facing new challenges, larger “caseloads”, telephonic visits, people who have never been connected with the behavioral health system and providers who cross the disciplines of health, substance use and mental health. Should we be training specialists or generalists? What decides the amount and intensity of contact? How do we handle ourselves with primary care and substance use providers? What about employment and housing issues? A paradigm shift requires us to do business differently. The webinar presenters will address all these issues and give us an opportunity to explore the real world implementation of care management by offering their perspective on these issues as well as lessons learned from the field.

Dale Jarvis is the founder of Dale Jarvis and Associates LLC, a Seattle-based consulting firm. Mr. Jarvis has extensive experience helping health plans and healthcare providers redesign their organizations to achieve better health for the populations they serve, better care for individuals, and reduced costs. He has contributed articles to publications and is a co-author of two books on healthcare system redesign. Currently Mr. Jarvis is focusing on healthcare reform to help ensure that the needs of persons with mental health and substance use disorders are addressed as reform unfolds.
Danika Mills is the Acting Executive Director of Coordinated Behavioral Care, Inc., a lead Health Home covering the boroughs of Brooklyn, Manhattan, and Staten Island in NYC, with a partnership with Bronx Lebanon in the Bronx Health Home. Ms. Mills was the lead executive during the startup of the Pathways to Wellness Brooklyn Health Home. Before the advent of Health Homes, she directed one of the six NYS Chronic Illness Demonstration Projects that piloted care coordination for Medicaid recipients with complex disorders, as well as led a SAMHSA project two Brooklyn shelters that successfully engaged chronically homeless women in services using evidence-based practices, winning a SAMHSA Science to Service Award. She is a NYS-licensed social worker and art therapist.
Early in her career, Sue McKenna was part of the initiative to provide comprehensive case management services to individuals returning to the community from state hospitals. Following a long career as a provider of CDT, Partial Hospital, Clubhouse and Supported Employment programs on Long Island, Sue was fortunate to be part of the Chronic Illness Demonstration Project as the Director of the initiative at FEGS. As CIDP transitioned into Health Homes, Sue assumed the role of Senior Director of FEGS’ phase I Health Home, Nassau Wellness Partners, consisting of 7 CMA partners and a diverse and growing group of community providers /network partners. Sue McKenna is a licensed Social Worker.
