NYAPRS Note: OMH has just announced the release of the HCBS provider application form, which allows providers to attest to provide new HCBS services under a HARP managed care plan. As the instructions indicate:
“…HCBS provider designation confirms that your agency has attested to provide HCBS within the
agency’s scope of practice and consistent with the criteria articulated in the manual. Your
agency will only be designated to provide the HCBS that are included within your application
and approved by the state. HCBS provider designation does not guarantee that your agency
will gain business for these services, nor does it mandate your agency must provide the
designated services.
…The provider Attestation is an executive declaration that the organization meets the
requirements to provide HCBS. Only one attestation form is necessary per applicant,
regardless of the number of services you are applying to provide.
You must complete an individual application form for each service you intend to provide. The
application is designed for providers to demonstrate that they have the organizational capacity
and culture to provide one or more of the HCBS. Applications will be reviewed based on an
Agency’s staff qualifications, experience, and ability to meet HCBS criteria.”
The rates for the HCBS services have not been released, but will be forthcoming. Providers should prepare their application regardless of the release of rates, recognizing that this application and subsequent designation does not mandate providers to offer these services. The deadline for submitting the application form to the state is December 5, 2014.
