Did you hear? Care Transitions Network Named a 2016 CQI Project!

The New York State Office of Mental Health has named the Care Transitions Network for People with Serious Mental Illness as a project option under its 2016 Continuous Quality Improvement (CQI) Initiative. Freestanding clinics that select the Care Transitions Network as their 2016 CQI project will not only reap the benefits of Care Transitions Network offerings, but also receive an enhancement to their clinic base rate for both Medicaid fee-for-service and managed care payments. CQI project selections must be made by August 15^th.

(Don’t forget to also have at least two clinic staff register and participate in one of the Office of Mental Health’s four CQI Project Overview webinars in the month of July; this is required to enroll in any of the three CQI projects.)
