Addressing Social Determinants of Health: Connecting People with Complex Needs to Community Resources

Complex care providers are increasingly aware of the social and environmental factors that contribute to poor health outcomes. In response, many providers are establishing connections to an array of community resources (e.g., food banks, employment services) to support peoples’ non-medical needs. This webinar will feature speakers from AccessHealth Spartanburg in South Carolina and Petaluma Health Center in Northern California who will share strategies to address social determinants of health through screening approaches, innovative technologies that track social service referrals, and strong connections with community partners.
Providers, health system leaders, and other interested stakeholders are invited to join this 90-minute webinar made possible through support from the Robert Wood Johnson Foundation. The webinar is second in a two-part series from Transforming Complex Care, a national initiative led by CHCS that focuses on advancing innovations in complex care.
