Join us next Tuesday, July 22, 2025, from 4-6pm at Brooklyn Borough Hall (209 Joralemon St, Brooklyn, NY) where you will hear directly from PHS’ WholeYou NYC,
the Social Care Network for Brooklyn, Manhattan, and Queens.
This session is designed especially for people enrolled in Medicaid, their families, and community members, but it is open to all, including peer specialists, advocates, and providers who want to better understand how these services may help the individuals they support.
At session’s end, a light meal will be served and a free scholarship
to the Alliance Annual Conference will be raffled off.

Background on the NYS 1115 Waiver:
Medicaid Demonstration Waivers are tools states use to test funding for services not traditionally covered under Medicaid. They allow for experimentation and innovation in how care is delivered and how health outcomes are improved.
New York’s newly launched 1115 Waiver is focused on reducing health disparities and expanding access to health-related social need (HRSN) services — supports like nutritious meals, housing help, care coordination, and transportation assistance that directly impact people’s health and well-being.
At the center of this initiative are Social Care Networks (SCNs) — regional networks of community-based organizations and providers working together under a lead agency to deliver these vital services to eligible Medicaid members.
While the waiver program is complex, this information session will offer a clear, helpful introduction to how SCNs function, what services may be available, and how to access them.
