This brief 5-minute survey—created by Cantata Health Solutions, a leading provider of technology solutions for behavioral health and human services—seeks feedback from individuals living with mental health and/or substance use challenges about how digital tools and patient portals could be improved to make care more accessible, personal, and empowering.
